Almost a disaster, what did I do wrong?

Yesterday was my first time using a PC polisher to apply polish to any car. I was purchased several pads to use and only had Menzerna SF4000. This was very much a learning experience for me. While using one of the Lake Country tangerine pads, I had only completed two panels with the pads and a HUGE chunk of the pad broke off from the pad leaving a bare spot in the pad all the way down to the Velcro connected to the backing plate. Fortunately I saw a few big chunks of tangerine pad fly off before the big chunk did so i was already pulling the buffer off of the car before the big chunk completely broke free (no damage was done to the paint THANKFULLY). None of the other pads i used had any problems (other tangerine, white and black pads). The PC was set at speed setting 6 at the time and I was working the passenger door.

What could I had done to cause this?

Thanks.

+1 for the defective pad theory. I wouldn't be using the polisher at its its higher speed settings until you get a hang of using it. I have a Porter Cable DA and seldom go over the speed setting of 4 out of 6. Just a suggestion.

I would also think it is a defective pad. Only time I've had that happen is on my cheapo throw away heavy cutting pads. I've yet to have issues with my Buff and Shine/Chemical Guys hex pads on my Flex.
