Help need advice from obd experts diagnosing a p0300

need advice from obd experts diagnosing a p0300

I know it's a LONG post, but please read it all, I can't help but think it's all part of the puzzle....
If anyone knows how to help me interpret these issues I've been having it would greatly help. I think i have almost all the issues resolved, but still have odd test results.
I have a 08 g37 6mt. with 220k on it, doesn't leak or burn a drop of oil, smooth as silk, and tons of power, but I've been getting a p0300 code that I couldn't shake. Checked afr trims, and when floored car was running afr 14 to 15, so it was lean. I assumed it was a lean misfire. Results were multiple misfires on 1 3 and 5 so bank 1. Replaced front afr sensors with aftermarket but new nipendenso sensors, and got the lean fixed. Now when floored I'll dip into high 12s. This also fixed my multiple random misfires. I would still monitor a few random misfires each drive, but now on 1 THROUGH 6 but no more than 3 misfires calculated per cylinder on a 40 mile drive. This essentially cleared the code and the car was happy. Fast forward to cold weather, and on cold starts I will get random multiple misfires, that jump around. One morning 5 and 6 will misfire 20 plus times, and another morning 1 and 3 will be the big misfire. Point is, it jumps around, and isn't specific to a certain cylinder. Once the ambient temp gets warm, I will have only 3 misfires again. (Randomly and on no specific cylinder)
If it's above 50 degrees here when I first start the car, I'll pass, and the computer is happy. On a 35 degree morning though as soon as started I get enough misfire to store a pending fault. I'm not sure how to interpret my ltft and stft, hence my post here. I suspect I still have a fuel issue. Ltft is 9.4 in both banks, and stft1 (at screen shot) was -3.1 with sweep range of -2.5 to -6 and stft2 @ screen shot -6.2 with sweep range of -5 to -8 and they would try and chase each other, but I'm afraid they are to far off and, however what is not normal, is for whatever reason my car will not initiate, (or dependent test failed) fuel bank monitor, bank 1 and bank 2. My concern is somewhere on cold start a sensor isn't doing its job and I get multiple misfires due to incorrect fuel. My afr at idle is perfect however at 14.7
Btw, these misfires can rarely be felt, and are only offensive when cold outside, so I have no clue. Tried cleaning maf sensors with hope that would clear it up, but no luck.
I know miles are high, but car runs great, and I have no other codes. No exhaust leaks I know of, and running test pipes, because theory was my converter was cracked. (That wasn't the issue) if there is a data log I can send let me know, (if you can and don't mind helping) I have been emailing myself short trip logs for the last few weeks with outside temps. I know that fuel bank monitor SHOULD be showing results for bank 1 and 2 but it never initiates (or fails and doesn't test)
Sorry long winded, but it's an odd problem. Any help would be great! Btw car is bone stock with exception of test pipes, no vacuum leaks found, and no oil leaks, including valve covers to coils.....no other codes thrown.

What maintenance have you done to the fuel system over the years if I may ask? (other than replace the 02 sensors)

Injectors are original, new fuel pump (due to another misdiagnoses) and constant hwy travel, with fuel injector treatment run 3k miles ago. Throttle bodies pulled and cleaned, and new ngk plugs 15k miles ago.

Is the fuel filter integral to the pump on our cars? Or is it inline somewhere? But...that many miles on the stock injectors, they're ready for a rebuild! Make sure the o-rings are all intact, they're sealing on the rubber seats, stuff like that. Could be a fuel pump regulator in need of replacement, vacuum leak to the regulator or elsewhere.

I could believe a regulator, I believe the filter is in line with the pump, so new there.
what could cause the fuel bank monitors tests to not initiate?
is the regulator easily replaceable? I believe I could check fuel pressure at the regulator, and if out of spec, go from there, correct?

I did a bit of research and the the filter is part of the white plastic surround that holds the pump. So if you replaced with an OEM pump, then you should be good.

My guess would be a regulator or some sort of vacuum leak after the MAF sensors. 200K of engine heat does a lot of damage to rubber parts. If the rubber gaskets that seal the injector tips to the head are worn/cracked, could cause a leak. Same with all the rubber hoses, o-rings, etc...I don't believe too much in fuel injector cleaner. You could invest in another set of OEM (new or low mileage used) injectors or pull yours and send off to RC Engineering or similar to be cleaned/rebuilt.

Understood, and noted. Thank you. Does anyone know what systems must be online to initiate the fuel bank monitors? What does this test actually show? What is going on that I'm failing or not testing. I know part of it means to test it needs good evap system, and I have that, good o2 sensors that check, I have that, and no other failed tests or cel....

As you probably already know your LTFT are a little high. However total fuelling is ok (9.4 -3.1 = +6.3) If you have the freeze frame data (mode $02), please post it.

When you cold start the car, is the idle more than 1,000 RPM? Does the OBD data show the coolant temp to be roughly accurate? (compare outside temp to engine temp on a cold start)

MAF check:
If you can log date, what is your MAF airflow at idle/every 1,000 RPM and under full throttle?

PCV Check:
Do a very close inspection of the PCV hoses and fittings. Any crack can allow unmetered air to enter causing a lean condition.

Fuel Pressure Check:
I know you have a new fuel pump, but it wouldn't hurt to check the fuel pressure at idle. (I think is should be around 51 psi)

The fuel system monitors are continuously monitored, so they run all the time. I assume your is failing all the time, but no code is being set which in weird.
Do both banks stay in closed look when at idle and with mild acceleration?

Your test pipe does add some additional variables. Newer cars with an air/fuel ratio sensor use the rear o2 sensor to determine if the front AFR sensor has any lean/rich bias. It is called "FAOSC – Fore/Aft Oxygen Sensor Control". Basically it looks at the delay of how quickly the rear o2 goes lean after the throttle closes. In your case, it will be almost instant, and that may be a problem.

How long has the test pipe been on?

How odd, I let the car sit for 2 weeks, went to start it hoping to exaggerate the issue. No misfires at all when cold and only a few while driving. Will retest when cold again, for now will answer what I can.
idle cold is 1000 rpm, and open loop. Engine temp after 30 seconds (it took that long to get the damn software stable) was 89 degrees both stft had no reading
hot idle is 650 to 675 and closed loop. Coolant 195
system is closed loop until floored or deceleration and goes to open loop. Stft 1 and 2 have bigger differences that each other, especially at idle, but do try to chase each other, and seem much more well behaved when driving .
at screen shot stft 1 was -12.5 and stft2 was-6.2
I'll try and post data next....

results from two weeks ago and 36 degrees

Here are results from two weeks ago. I took the results as soon as torque would initiate all the tests, so these results are about 5 minutes after start up, no driving. The misfires did not climb significantly when driving, so essentially these misfires happen at idle cold start. Once hot, the most I record are around 4 misfires in a 40 mile trip. See below:
